CMS New Patient Packets
The Forum of ESRD Networks sends out a New Patient Packet to all new dialysis patients following their registration through the completion of a CMS 2728 form. It takes approximately 2-3 months for the packet to be mailed.
It includes:
A letter from The Renal Network with a list of vocational rehabilitation offices and contact information for Network resources.
A National Kidney Foundation (NKF) brochure entitled The Voice, The Home, The Hope, which gives organization and membership information
An AAKP resources pamphlet with membership information;
And the following CMS material:

The Renal Network encourages each facility to have their own new patient packet that can be given to patients and their families soon after starting on dialysis. The facility new patient packet should include a copy of the facility patient rights and responsibilities as well as the facility grievance policy.
