This color-coded map of the four-state Network area (Illinois, Indiana, Kentucky and Ohio) summarizes the prevalent fistula percent rates for ESRD Network 9/10 by Health Service Areas (HSA) for the month of December 2007. 

The colors reflect a scale of progressive benchmarks determined by the Network Medical Review Board to assess progress in meeting the Network increased placement of arteriovenous (AV) fistula goals. 

The prevalent fistula scale used in the chart is:

Less than 40% equals purple
40.0% - 50.0% equals blue
50.1% - 58.0% equals orange
58.1% - 65.9% equals yellow
66.0% and greater equals green
